Fourth year child and adolescent psychiatry residents are involved with the day treatment program for about five months. The day treatment experience occurs at the Fairview-University Medical Center, Riverside Campus. One half-day (four hours) per week is scheduled for day treatment related activities, which include rounding on patients, interdisciplinary team meetings, and supervision. The trainee is asked to be available by pager to the interdisciplinary team for medical/psychiatric support and to evaluate new patients within 24-48 hours of admission to the program. Activities include evaluations and treatment of children and adolescents who are admitted to the program, medical/psychiatric management of patients, and leadership of the interdisciplinary treatment team (therapist/case managers, occupational therapists, schoolteachers, and dieticians).
On-site individual supervision of the trainee is provided during the scheduled half-day per week. This supervision generally includes direct observation of trainees interviewing the child or adolescent, discussion with the trainee of differential diagnosis and formulation of the case, participation in presentation of findings to the interdisciplinary treatment team and providing recommendations for a treatment plan. Faculty supervision is available throughout the remainder of the week as needed. In addition to the supervision provided by the faculty, the interdisciplinary treatment team offers insight and guidance to the resident with their contributions in patient evaluations and treatment plans.
Day treatment population averages 70% male and 30% female. Caucasians account for 85% of the patients seen. The age group of child and adolescent psychiatry patients ranges from preschool to 18 years of age. There is a wide range of diagnoses for patients seen in the day treatment program such as developmental disorders, disruptive behavior disorders, affective disorders, psychotic disorders, adjustment disorders, substance use disorders and eating disorders. On average, there is one new admission per week and the average number of patients seen at any one time is six. Referrals to the program come from the Fairview-University Medical Center (FUMC) inpatient psychiatry teaching service, the FUMC Child and Adolescent Psychiatry Outpatient Clinic and community psychiatrists.
